The SHOTS survey is appropriate for use as a self-administered questionnaire or in interview format. The instrument takes approximately 5-10 minutes for self-administration. No special training is required.


Scoring Title

Each item is rated on an ordinal scale from 0 to 4 reflecting the degree to which the item is considered a problem for parents or primary caregivers related to childhood immunizations. The parent or primary caregiver responds to the question

"Below is a list of things that may cause problems for parents getting their children immunizations. On a scale of 0 to 4, with 0 being "not a problem at all" to 4 being a "very big problem", please CIRCLE your answers".

The options for responses are 0 = not at all, 1 = a little, 2 = somewhat, 3 = quite a bit, 4 = a lot.

A total composite barrier to SHOTS score is obtained by adding together the items. The range for the total SHOTS score is 0 to 92. The range for the Access to Shots subscale is 0-48, the Concerns about Shots subscale is 0-24, and the Importance of Shots subscale is 0-20. The higher the composite scores, the more problematic that group of items are for parents getting their children immunizations.

To compare the subscales with each other or the total scale to the subscale, calculate the mean for each subscale by dividing the composite score by the number of items making up the scale.

  • Access to Shots subscale, divide the total composite score by 12;
  • Concerns about Shots subscale, divide the total composite score by 6;
  • Importance of Shots subscale, divide the total composite score by 5.
Fees
